Please fill out the application form (all questions are mandatory unless otherwise specified) and mail it along with the Annual membership fee (a $50 check payable to GDP-WASH) to the following address:

The check with the Annual Membership fee will not be deposited until the applicant becomes a member of the organization. Membership is granted only upon final Board approval.
